At Fitzpatrick Home Care, we are committed to making a positive difference to the lives of the people we support.
We understand that every client is unique. Each person has their own routines, preferences, needs and goals, which is why we take time to provide care that is personal, respectful and tailored to the individual.
Our aim is to support clients to remain as independent as possible, while feeling safe, comfortable and listened to in their own home. We work closely with clients and, where appropriate, their families, to provide a thoughtful and reliable home care service.
We also support and value our care team, helping them deliver care that is safe, compassionate and professional.
-
Arranging care begins with a simple conversation.
You can contact Fitzpatrick Home Care by telephone or email to talk through your needs or the needs of someone close to you. We can then arrange a home visit to get to know you, understand your circumstances and discuss how we may be able to help.
There is no pressure to make a decision straight away. We will take the time to answer your questions and explain the options available, so you can make a choice that feels right for you and your family.
-
Before care begins, we will agree a personalised care plan with you. This plan sets out the support you need, how you would like your care to be delivered, and the outcomes that matter most to you.
Your care assistant will follow this care plan during each visit. If your needs change over time, we will review the plan with you and make any appropriate updates.
Tasks may include:
Personal care and toileting support
Medication prompts or support, where agreed
Meal and drink preparation
Support with laundry and light housework
Shopping or help with everyday household tasks
Companionship and reassurance
Any tasks will be agreed with you in advance, and changes will be clearly communicated to your care team.
-
There are some tasks our care workers are unable to carry out, usually because they require clinical training or may present a risk to the client or member of staff.
For example, our staff are not able to give injections, change sterile dressings, or cut fingernails or toenails.
We will always be clear about what support we can safely provide. If a task requires help from another professional, such as a district nurse, chiropodist, GP or other healthcare provider, we can help guide you in the right direction.
-
We understand that care needs can change.
If your current care package no longer feels right, or if you would like to make changes, please contact us. We will listen to your concerns, review your care plan and discuss what adjustments may be needed.
Where appropriate, we can also involve close family members or representatives in the review, so that everyone feels informed and reassured.
Our aim is to make sure your care continues to be safe, suitable and supportive.
-
We will always try to provide consistency wherever possible, as we know how important familiar faces can be.
However, it is not always possible to have the same care worker every day. This is because staff may be on holiday, off sick, attending training, or supporting other clients.
To provide reliable and continuous care, we usually introduce a small team of care workers who can get to know you and your preferences. This means you are still supported by familiar people, while ensuring your care can continue safely when individual staff members are unavailable.
-
Yes. We take care planning seriously and will agree the tasks to be completed within each visit before your care begins.
Your care assistant will follow your care plan and record the support provided after each visit. This helps us monitor the care being delivered and check that your needs are being met.
If you feel that the visit time is no longer enough, or that additional support is needed, please let us know. We can review your care package and discuss whether any changes would be helpful.
-
The cost of care depends on the type and level of support required.
After discussing your needs, we will prepare a detailed care plan that explains the care to be delivered and the associated costs. We aim to be clear and transparent, so you understand what is included before care begins.
Some people may be eligible for financial support through their local authority, Social Services or other funding routes. Where helpful, we can signpost you towards the relevant organisations for advice.
-
We will send you an itemised invoice every four weeks.
Payment options can be discussed with the office, and we will explain the available methods clearly when your care package is arranged.
If you ever have a question about an invoice, please contact us and we will be happy to help.
-
Your safety and peace of mind are extremely important to us.
All Fitzpatrick Home Care staff complete appropriate recruitment checks before they begin working with clients. Care assistants wear uniform and carry identification so that you know who is visiting your home.
If you are ever unsure about the identity of a care worker, or if something does not feel right, please contact the office before allowing entry. We will be happy to confirm who is due to visit.
-
Our office is open from 8:30am to 5:00pm, Monday to Friday.
Outside office hours, including weekends and bank holidays, we operate an out-of-hours on-call service from 6:00am to 11:00pm. This means there is someone available by telephone to help with urgent matters relating to your care.
The out-of-hours contact number will be provided to you for reassurance.
